U/Uploaded PDF Form

The U/Uploaded PDF Form is a place where supporting documents can be shared in an event. 

1. Click Create Form
2. Select the U/Uploaded PDF form
3. Click Create 

1. Add the description of what is being uploaded
2. Select the PDF file from the computer
3. Click the format the forms should be saved as

The U/Uploaded PDF form will include the additional title which was added.
